Duplicate Birth / Death Certificate

Requirements:

To obtain a copy of birth/death certificate, a search form must be completed and returned to the Kenya Embassy D.C.
Complete the Birth/Death Certificate Search Form and submit the completed form with the following:

1. 1. 870 Mexican Pesos Money order or Cashiers Check Payable to Embassy of The Republic of Kenya.
   2. Copy of the original Birth/Death Certificate.
   3. A formal letter of Application.
   4. A self-addressed stamped envelope for the return of your documents. The envelope should have enough postage for DHL (NB: Metered stamps and Regular mail are not acceptable).

When completed, this application should be sent to –
